Make a form people can fill in on screen
Paper-style forms are slow to fill in and hard to read when they come back. Create PDF Form lets you draw real fields onto any page: an application form you exported from Word, a scanned registration sheet or a template you designed. The people you send it to can fill it in with Acrobat, Preview, their browser or this site, and you get clean, typed answers back.
Each field gets a name, which is what form-processing software uses to read the answers. Radio buttons with the same group name work together so only one can be chosen, and dropdowns take a list of options, one per line. Mark fields as required where your workflow needs them.
